PSMC integrates the results of environmental safety and health risk assessment with the concept of a Three-Sections and Five-Levels prevention, and adheres to the implementation model of PDCA and the spirit of continuous improvement to plan complete health checkups and health promotion activities, and actively participates in the health promotional activities organized by the Ministry of Health and Welfare, the Public Health Bureau of Hsinchu City, the Ministry of Labor, and the Hsinchu Science Park Administration to build a healthy and friendly workplace environment with a systematic health management model and diversified resources.

In addition, PSMC followed the Operating Rules for Contractor Work Permit Application to compile statistics on the contractor personnel such as outsourced security personnel, cleaners, staff canteen workers, etc. who performed in-plant duties for 3 or more months in the past year and shared the same work environment or had regular work contact with PSMC employees. For such in-plant personnel, contractors have been required to provide their health check reports or a health declaration issued by the contracted companies on a yearly basis so that the Company can keep track of the in-plant personnel's health status. Those who do not provide health check reports or explain reasons shall be forbidden from entering PSMC plants. The control achievement rate was 100%.

Based on a human-oriented business philosophy, PSMC has established the Three-Sections and Five-Levels Prevention Strategy to develop a full range of health check campaigns and conduct a series of health promotion activities, for which the health management budget and resource investment have been increased year by year to ensure the concrete implementation of the health management system for employees to receive the most timely and appropriate improvement treatment. PSMC's health management system is carried out through "advanced physical examination, general physical examination, physical examination for special operations personnel, new employee physical examination, foreign worker physical examination, and contractor physical examination", which is then reinforced through different levels of health management measures and anomaly tracking and improvement mechanisms for the Company to stay on top of employees' health status. Case tracking shall be conducted for personnel of high-risk groups such as workers engaged in special operations, etc. The cases shall be required to receive rechecks, and will be assisted in visiting Powerchip Clinic or onsite occupational medicine specialists to receive consultations, thereby enhancing their awareness of health management.

Health management is achieved through physical examinations and other regular health exams, to stay abreast of workers’ health. Through classifying health statuses and diseases, we can manage and track; we also arrange consultation with occupational physicians, offer health education, provide health education instructions, and implement health promotion activities

Since 2007, after taking into consideration advice from professionals, the Company reviewed the operations of the fabs and undertook consolidation and planning, which ultimately led to the establishment of the Occupational Disease Preventive Measure Management Items. Via (occupational disease) Prevention, Return to Work and Compensation (PRC), the Company strives to achieve its ultimate goal of employee health care—— No Pain at Work, Healthy Life's a Perk.” Meanwhile, we also reviewed potential hazardous operations, updated the list of these operations, and incorporated them in the internal supervision management with a multi-layered preventive method and diverse proactive management.

Providing more than what the law stipulates, PSMC sponsors comprehensive medical examinations for its employees annually and will follow-up on various medical abnormal items, so that early treatment can be provided upon early detection. Moreover, the employees who receive health examination will be classified into different health levels for health management by the health center in accordance with the results of their diagnosis at the hospital:

Based on the results of the employees' annual health checkups, PSMC assess the types of health promotion needs through various activities such as ultrasound examinations, vaccination, low-calorie healthy meals, cardiovascular group care, and 3-point checkup for women. PSMC also hold various seminars and provide health professional consultation services to enhance the employees' cultivate correct health education concepts. The specific coverages are as follows:

• Evaluate health examination data such as employee health examinations and medical examinations for new employees, analyze items with high abnormality rates, and plan them as key tasks in the health promotion plan.
• PSMC use the Employee Health Needs Questionnaire to understand common health problems and implement various health promotion activities such as stress management, physical fitness activities and dietary guidance.
• In accordance with laws, seasonal and employee needs, PSMC post new health information (divided into three categories: safety and health newsletters, general medical knowledge and health education for chronic disease groups) to enhance employees' general health knowledge.
• Through medical checkups of staff and newcomer, those with chronic diseases are screened and health education pamphlets are regularly sent every three months to keep employees up-to-date with the latest health education knowledge.

In order to meet the medical service needs of employees and contractors at work, PSMC has set up the Powerchip Clinic within the plant to provide health management services such as outpatient consultation, health counseling, and preventive vaccination. Relevant health information and activities are available on the company's intranet "Health Management System" for inquiries and appointments. In addition, the clinic has also appointed on-site occupational medicine specialists to provide health education, guidance and planning, and assist in employees' fitness-for-work arrangement. In view of shift work type (four shifts and two rotations), in addition to the full-time nurse practitioner at Powerchip Clinic, there is another nurse practitioner on duty 24 hours a day in the plants to provide professional health consultation, health promotion activities and emergency care services to ensure the safety of employees.

Via an electronic health management system, the employees can not only see their medical examination reports over the years to have a full picture of their health condition, but also make appointments with doctors, sign up for health promotion activities, and consult physicians about their health problems online. The system also offers various and comprehensive health services to strengthen our employees' abilities for health self-management.

• We update the bulletin boards on a regular basis, post health information and health education posters and so on.
• We e-mail our employees on the different types of health promotion activities and health information messages for their reference.

In response to the possible impact of COVID-19 on the Company, since the end of 2019, PSMC’s Risk Management Office has continued to pay attention to the development of the pandemic, and began to conduct appropriate health education for employees to avoid misunderstandings. When the pandemic is worsening, the team is activated to join the pandemic prevention work according to the classification of the pandemic situation, and the measures are gradually implemented accordingly. The measures will be adjusted appropriately with reference to ISO45005 Guidelines for Managing COVID-19 Risks.

• Powerchip CDC raises the level of commander in response to the severity of the epidemic. The top commander is the company’s President.
• The mobile/computer version of the Pteam APP is a platform to connect various electronic systems, develop various epidemic prevention functions, avoid clusters and implement comprehensive health risk management.
• Employees conduct self-health management and cooperate with the government’s pandemic prevention measures. If necessary, employees conduct self-monitoring and wear masks. Powerchip CDC will track the cases. Employees with a high risk of infection are listed as health tracking targets and PSMC will provide relevant assistance. In addition, in order to avoid physical discomfort and psychological panic caused by facing the unknown, the counseling & psychological construction/advocacy will timely counsel/assist all employees.
• Strengthen environmental cleaning and disinfection in the public areas and add disinfection supplies for employees to use. Conduct body temperature check and pandemic prevention information questionnaires for all incoming manufacturers/visitors.
• Employees inform vendors on the relevant epidemic prevention measures and to wearing masks all the time. Everyone avoids unnecessary travel and face-to-face meetings or decline visitors from high risk areas. High-risk groups are required to provide negative test certificates (including household rapid test)/vaccine certificate to enter into the office.
• Plans of work off-site, work from home, or video conferencing, etc. are implemented to reduce the risk of personnel exposure to infection.
• Start various operational impact preparations, including supply chain confirmation, product entry and exit disinfection, product inventory storage, customer communication instructions, and media communication preparations, etc.
